Install Fusion Mod and use the launch option I used. This mod gives a lot of QOL improvements and also lets players use the DLCs Ubisoft has barred and makes it easier to play multiplayer through unixoide/5th-echelon.
Considering the state of the gamer after Ubisoft shut the servers, the mentioned mod and tinkering steps are necessary.

The configuration you have to figure out is kinda a pain and unfortunately Ubisoft would rather keep that 30m bug in and not fix it

ran it with the -offline -offline_mode flags to try to keep it from crashing. Unsure why but when run without those flags I would end up getting an error Unable to find UbisoftConnect.exe, even after all this it still had the crash every 30 minutes bug
Multiplayer servers I believe are down, however splitscreen co-op runs just fine.
Overall, 2/10. Gameplay itself runs pretty well, but I stll hadn't tested the game fully yet on other ideas

The game runs great it works right out the box using proton 9.0-2. You can install "Non Steam Launchers" app which will install Ubisoft app. This will allow to you to log in to start the game.
There aren't any issues.

Like many I experienced random crashes 20-30 minutes into my playtime despite the game running very well... but i figured out the fix! In desktop mode, find 'proton experimental' in your steam gamelist and open properties. Go to Betas and select 'bleeding-edge-latest and untested dxvk, vkd3d-proton and wine changes; backup your prefixes before using'.
As suggested, this will alter your prefixes that you may use on other games; but since doing this the game runs absolutely flawlessly with no crashes. This is because this version of experimental utilises dxvk, which blacklist uses.
Of course, make sure you then select and use 'Proton experimental' for Blacklist itself otherwise nothing will change.
DLC RESTORE

The save files are located at the following location in dolphin (steam version)

This may differ for you for whatever reason, but this is where I found it.
Simply replace the files. Back up your previous saves if you want.
Upon replacing the save files, the dlc will be restored. And yes, you can play the game with the DLC from the beggining after loading up and hitting 'Restart story' in the pause menu settings (While in paladin).
As for multiplayer / Co-op it simply doesnt work anymore, not even splitscreen like on the console versions of the game, a real loss. PS: Sometimes the game works 100% completely offline, but this is tempremental, it may or may not ask for your ubisoft connect password; anticpate that you might need to be online on the inital game startup, afterwhich disconnecting/ going into sleep mode shouldnt matter.

Go into desktop mode and put ubisoft connect into offline mode this will remove the need to enter your password every time you launch game making it work in game mode due to the login page braking in game mode.

Basically works out of the box with the .deb based Steam install using Proton 8 once you figure out how get past the start-screen crash.

If I don't alt-tab out during start-up, the game crashes when moving past the starting screen. If I alt-tab out and back in, no problems at all.
I haven't tested if I the -offline -offline_mode flags are needed or do anything, but I've played the game 4+ hours straight and never had a crash while using them so...

I was bad and sailed the high seas, swapping the game folder with a pre-installed copy from SteamUnlocked. This allowed me to bypass the s*** uplay launcher and theoretically get better performance with the extra DRM removed.

This is an update to an old report made months ago. Installing CryoUtilities and using the recommended settings including setting the VRAM alongside the info previous listed, this now runs at a very solid 60fps in game where previously it would not.

When launching the game, you will be required to login to Ubisoft Connect. For some reason, being on wifi will get you an error when logging in. Using a mobile device to set up a mobile Hotspot fixes this issue and logs you in. This is only needed for the initial login.

Cloud syncing of saves does not appear to be supported.
Tom Clancy's Splinter Cell Blacklist is a typical old Ubisoft game with CPU affinity problems. It only runs on core0 (this can be fixed with the taskset -cp command) and seems to use a maximum of two cores when changing the cpu affinity. Unfortunately, this has to be done when the state of the process (application) changes. For example, when a cutscene is played, the process goes to sleep. After the cutscene, the game only uses core0 again. This can be monitored with btop or htop and mangohud.
The game can be played without using the taskset cpu affinity fix, but will have worse frametimes (atleast on my system).
As listed by other users, the following fixes needs to be applied:
- Deleting or renaming systemdetection.dll file
- Using the offline command "-offline -offline_mode"

Cannot get past uplay login screen. Loads for a long time and always ends with a connection error.
Unplayable for me, I tried many versions of proton with no success.

A commandline prompt appears and logs thing, but it was easy to switch windows with no trouble thereafter.
I'm using the ubisoft connect download of this game. For this to work I had to point my steam shortcut at the games exe directly otherwise the launch options wouldn't work (I believe this is more of an ubisoft connect issue). I also need an internet connection to launch due to ubisoft asking for my password at launch, however this is the same for my windows install. I've played a few hours now with no noticable issues to the game itself once launched and no issues with steam deck functionality.

I'm changing my report after finding some new information. I have found most of my Ubisoft game issues stem from my CPU having too many cores. I tried the fix that I mentioned in my Far Cry Primal report, but it didn't help Blacklist as much. Still experienced video and audio stuttering. However, based on a report for Splinter Cell Conviction, I tried a similar cpu fix that ended up working great. The script is here.

If MXAA 2x or 4x is used for anti aliasing in the video settings, I got artifacts in the sky as mentioned by @apoorv569 here. SSAA 2x2 Rotated Grid worked fine.
Standard Uplay issue of requesting the password every time, and not exiting (have to quit from my system tray). I tried unchecking Uplay option "Minimize Ubisoft Connect to the system tray when the application is closed", but it didn't help.

In order to get this to run (on a nvidia gpu) I had to combine several of the fixes below. This is what finally worked for me:
- Set launch options: -offline -offline_mode "-uplay_steam_mode"
- Browse to local files and rename Tom Clancy's Splinter Cell Blacklist/src/SYSTEM/systemdetection.dll to something else (I just changed it to systemdetection_.dll)
I thought I had to also change compatibility from Experimental to an older version, but then I put it back to Experimental and the game still launched fine.
Finally!! And like others have stated, after the game exits I have to press the "Stop" button in Steam to kill it.

Unfortunately this game is not really playable on Steam Deck.
I was unable to launch the game on the latest version of Proton. GE Proton and Experimental did manage to launch the game. It would run great and be a lot of fun until you get a hard crash after aorund 20 mins of gameplay.
This time frame varies, but it's guaranteed to happen sooner or later and it's can be very frustrating to crash during an exciting moment. Pretty disappointing.

First problem
First it doesnt start so I switch to older version of Proton (6.3-8) and then this error ocurred The procedure entry point cef_browser_create_sync could not be located in the dynamic link library libcef.dll

Next I try to reinstall Uplay service, because I have read that steam installed older version of Uplay. I downloaded installer from Ubisoft connect and install it in wine prefix like this.

Second problem
I run into vulcan error: terminate called after throwing an instance of 'dxvk::DxvkError
Solution
I solve it by installing all vulcan drivers/libs for my amd card.

I haven't installed lib32-vulkan-radeon. This solved all my problems, and game run just fine.
The only mode that works is single-player. Adding the offline and uplay launch options works around the 30 mins crashes.

The game needs to be installed on the local drive to work. With the frequent crashing, this game is barely to non-playable.
Audio Issues, 20 - 40 mins until application crashes

After intro mission, game crashes after loading 02_PostGamplay.bik and crashes if you delete/rename that file.
I've tried multiple proton and proton-ge version, editing the videosettings.ini, switching to DirectX 9, multiple fixes from the PCGamingWiki site, and launch options: PROTON_USE_DXVK=1 PROTON_NO_ESYNC=1 -offline -offline_mode gamemoderun %command%. Nothing seems to work. Borked. If it's not the .bik file then I think it has something to do with Uplay/Ubisoft connect.
Barely playable: even with Proton Experimental (tested January 4th, 2022) it still crashes every 17-25 minutes.
Still crashing after 17-25 minutes with either Proton Experimental or 6.3-8. No version of Proton seems to avoid crashes so far.

Switch out of big picture mode before downloading. When the download begins right click it in library/collection and properties/compatibility mode and use proton experimental. After it finishes downloading click the play button. It will begin installing ubisoft connect. It will make it to 100% downloading and then give an invalid entry point error. Disregard it and close it and the game out. Go back and click play again. This time the ubisoft connect will load perfectly. Create an account or login to one and enjoy your game. I agree with someone else this should remain a platinum game.

If you Shift+Tab to move your cursor into another monitor the cursor will never work within the game again. You will have to use wasd or arrow keys to restart the game.
Ubisoft shutdown the online services years ago- there is no multiplayer. Singleplayer works almost flawlessly. As others have stated: you will have to run the game multiple times when initially installing for Ubisoft to install it's accounts/programs/etc.
Worked pretty well, except for a surprisingly high CPU usage (40% on a hexa core)
Had to launch more than once for uplay to update and install ubisoft connect, also wouldn't start the game until i removed "Systemdetection.dll" from "...\Tom Clancy's Splinter Cell Blacklist\src\SYSTEM" directory, This isn't a linux / proton specific issue it happens to people on windows also(old problem it seems), once that's done it works.
Motion blur can be disabled in the steamapps/compatdata/235600/pfx/drive_c/users/steamuser/Documents/Ubisoft/Blacklist/videoSettings.ini (along with other settings, however it won't be there until you launch the game at least once). ubisoft connect needs to be closed manually after exiting the game but that's the same with other ubisoft titles(Far Cry 5/6). Recommend you keep it to 60fps and use vsnyc some in game physics bug out if you go above that.
