Works fine without any launch options and Proton GE. With Proton Experiemental, it would hang and trying to start the game with the spinning Steam logo on my Steam Deck.
Renamed the game exe to the launcher so it skips the stupid non-gamepad-compatible launcher.
mv Fallout4.exe Fallout4Launcher.exe
The initial splash menu comes up fine with mouse support, but the main game menu has no mouse cursor. Tried disabling splash intro page with editing the Fallout.ini file in the root directory and adding a 0 after both SMainMenuMovieIntro= and SIntroSequence= to no effect.
Skipped the intro splash screen with file renaming, but still no mouse on the main screen:

For SteamOS users (presumably due the the compositor "steamcompmgr") disable the initial splash screen with the following:

This will skip the splash screen. Before launching, ensure you use a Steam Controller profile that is for an XBox controller (no mouse/KB buttons/assignments). Controls worked great after this. I could not get KB/Mouse to work at any stage beyond the splash screen.
When going between menu items, mouse loses focus on my Steam Controller. Just have to be careful to keep the guns holstered if or figure out a "double press" option that does nothing to kick it back into focus if I pause the game.
The GPU pre sets (even after running a benchmark that ends super quick) to low. It seems when I set the graphics to Ultra, the quality really doesn't seem to improve much.
Crashes right after Marvel opening title splash.

Had to click X on origin mini-menu blocking the screen on Fullscreen for SteamOS/GamerOS
No large issues on SteamOS. 110 FPS or more. Had to turn on vsync to solve a bit of tearing at the top of the screen. Steady 60 FPS with vsync on an Nvidia 1080 graphics card.
