Works fine except for small input issue I mentioned earlier, which is annoying but not enough to have a significant impact on enjoyment.
On occasion (maybe for a few seconds every half hour or so), game will not pick up input from mouse or keyboard. It will pick it up again if you alt tab in and out, or if you just wait a bit. This problem was far more pronounced when I used an older version of proton, 4.2.
As mentioned on other reports, the following steps were necessary, but no other tinkering was required:
Browse local files, and open a terminal in that folder
Run the following command:
mv ./bin ./bin.bak && ln -s DefEd/bin bin && cd bin && mv ./SupportTool.exe ./SupportTool.bak && ln -s EoCApp.exe SupportTool.exe
Needed these launch options: "PROTON_USE_D9VK=1 %command% -useembedded"
Having to run in windowed mode is a little annoying, but not a big deal for a low res point and click game. Otherwise it runs fine.
On native version, cursor would occasionally drift and move erratically. But this only happened sometimes, and I don't know what triggered it.
On native version, on dual monitor setup, game would mirror to both monitors. I found that this effect persisted even when I closed out of the game, and I had to reset my display config afterwards.
On proton version, game would not start at all in full screen. But windowed mode runs fine.
Initially had issue described by other users where game would black screen upon saving. I was able to fix this by running "protontricks 250520 xna40" at the terminal.
The game will not stop running when you exit, so you need to manually kill underrail.exe. Otherwise it seems to be running fine after this fix.

The developers have said they are working on a native linux release, and that they'd hopefully have good news by the end of the year. If you're not in a hurry to play this game or if the glorious eggroll fork of proton doesn't work for you, then you can wait for that and hopefully have a more ideal experience.
