Had to sign in to my Xbox account in desktop mode
The sign in window goes into a loop in gaming mode
Microsoft Store and Steam save games do not automatically sync. You have to use a tool like GPSaveConverter on a PC to get your Microsoft Store save file over to the Steam Deck.

All audio stopped coming out of the left speaker. A game restart fixed it
I used IV Downgrader to downgrade the game to version 1.0.7.0. This is the guide I followed (https://steamcommunity.com/sharedfiles/filedetails/?id=2795496594). Add the downgrader as a non-steam game and use proton experimental on it. Be sure to do this in desktop mode since you'll need to get the path of GTA IV to downgrade it. I personally selected all mods except for the trainer. I would recommend to at least select the xliveless mod to bypass RGL, so you never have to deal with it again.
Mapped R4 to Left Stick for sprint
Kept the game on low settings and capped the refresh rate to 40hz
